T3SC (Tameside 3rd Sector Coalition) is dedicated to providing support, advice and information to our local voluntary and community groups (the VCS or third sector).
We are a registered charity (no:1094744) working throughout Tameside.
 


The Health and Social Care Network
A network for Voluntary and Community Sector Providers of Health and Social Care Services.

The Health and Social Care Network - Brings together groups and organisations in the Voluntary and Community Sector whose work has a health and social care focus. The network provides an opportunity for groups to share information about services, receive briefings and shared training and also develop a collective voice to influence statutory sector developments/service & strategy. Representatives from the Health and Social Care sit on the Health Partnership Board and other strategic groups.

The network meets quarterly. The Health and Social Care Network also links into Tameside and Glossop Health is for life festival (H4L) Contact Jane Friend.

Our Health, Our Care, Our Say-A New Direction for Community Services (White Paper 2006)

What’s it all about?
The Department of Health launched a white paper in 2006 called Our Health, Our Care, Our Say: A New Direction for Community Services. The white paper focuses on how health and social care systems will provide personalised and community-based services. The white paper also talks about giving service users a strong voice in the improvement of services as well as focusing on the reduction of Health Inequalities.

Priorities of the Our Health, Our Care, Our Say (White Paper 2006)

  • Better prevention services with earlier intervention
  • More choice and a louder voice for patients
  • Doing more on tackling inequalities and improving access to community services
  • More support for people with long term needs

How are the improvements going to be made?

  • Practice based commissioning - the devolvement of responsibility for commissioning services from Primary Care Trusts to GP practices.

  • Shifting resources into prevention - funding services, projects and initiatives which prevent health problems such as providing information and advice about physical activity.

  • More care undertaken outside the hospital and in the home - The provision of care locally and in convenient settings.

  • Encouraging innovation-development of new services based on what services people want.

  • Better joining up of services at a local level - Primary Care Trust and Social Care implementing a common strategy for providing services.

  • Allow different providers to compete for services - The third sector will be able to deliver primary care services.

Choosing Health-Making Healthier Choices Easier (White Paper 2004)

The over arching principles of Choosing Health

  • Informed Choice - providing information so that patients can make choices about their health.

  • Personalisation - ability to access services that meet the needs of the service user.

  • Working Together - The NHS, Communities, Local Government, the Voluntary and Community Faith Sector, Businesses to work together to provide health provision.

Overarching Priorities

  • Reducing the numbers of people who smoke

  • Reducing obesity and improving diet and nutrition

  • Increasing exercise

  • Sensible drinking

  • Improving sexual health

  • Improving mental health

 

The Tameside and Glossop local picture
Tameside and Glossop PCT Strategy.

In 2008-2009 Tameside and Glossop PCT will be working towards local and national priorities.  The priories are as follows:-

  • Alcohol Misuse
  • Cancer
  • Children’s Health
  • Dental Health
  • Management of long term conditions – Coronary Heart Disease & Diabetes
  • Maternity services
  • Mental Health & Learning Disabilities
  • Sexual Health
  • Stroke

Health Partnership Board-Link to Choosing Health 2004

The Tameside Health Partnership Board is partnership of statutory and voluntary community organisations working towards improving health and addressing health inequalities.  The partnership are focusing on the following priorities

  • Infant and maternal health

  • Lifestyles (smoking cessation, increased exercise, healthy diet and alcohol)

  • Environment

  • Targeting the over 50’s

  • Teenage pregnancy and sexual health

  • Mental health promotion

Health Inequalities Spearhead -Link to Choosing Health 2004

The Health Inequalities Strategy is working towards reducing health inequalities or the 'differences in people's health between geographical areas and between different groups of people' in the Tameside and Glossop area. The 2004-2008 Health Inequalities Strategy focuses on the following priorities:-

  • Smoking

  • Diet, nutrition and oral health

  • Physical Activity

  • Alcohol and substance misuse

  • Teenage pregnancy and sexual health

  • Mental Health Promotion

A new strategy is now in the consultation stage. Once this has been agreed and published a copy will be added to the website.
